Documents for a residence card

The most common reason for long waits is an incomplete set of documents. Build the list for your basis in the checklist below, and we'll check everything before filing.

Basic documents

Passport, online application in MOS, digital photo, proof of fee payment, health insurance, proof of address and income. Plus documents for your purpose: contract and Annex 1, university certificate, civil records or company documents.

Foreign documents

All foreign-language documents need a sworn translation into Polish. Records from abroad need an apostille or consular legalisation.

Fingerprints and collecting the card

After filing, the office calls you in for fingerprints. In Wrocław this is the Wydział Spraw Obywatelskich i Cudzoziemców, pl. Powstańców Warszawy 1. You collect the card in person after notification.

The certificate that replaced the passport stamp

Since 27 April 2026 the office no longer stamps your passport. After checking the application it issues a filing certificate (zaświadczenie o złożeniu wniosku), which you download and print from MOS. It confirms legal stay until the decision but does not let you re-enter Poland or travel in Schengen.

How to avoid a request for documents

Make sure the data in the form, contract and passport match exactly. Insurance must cover the whole period. Proof of address must be current. We check everything against a list before filing.

Filing deadline

File no later than the last day of your legal stay on a visa or previous card. You then remain legal until the decision, confirmed by the MOS filing certificate.
